We are now scheduling appointments for school entry physicals and vaccination updates.  CALL SOON to reserve your appointment 237-4423.   Please call early to avoid the summer rush!  Our office hours are Monday through Friday 7:30 a.m. to 4:00 p.m.

Kindergarten & Grade 6 admissions require physicals and current immunization records.  We also schedule Pre-School and Headstart exams.  We can bill Medicaid and Humana Insurance.  All others will be based on a sliding fee scale according to your household size.  Parent/Guardian must be present for the appointment.
